From the age of 16, patients are legally entitled to control access to their own medical records. This means that parents, guardians or any other third party cannot view or discuss a patient’s records without the patient’s consent.

Patients aged 16 and over can choose to give permission for others to access their information if they wish. Without that consent, medical staff are unable to share any details.

This policy helps protect patient privacy and ensures that everyone has control over their own health information.

If you have any questions or would like to set up consent for someone to access your records, please speak to a member of our team. If you wish to proceed and arrange consent please download the form below and return it completed and signed to the surgery so we can update your records.
