Temporary Freeze on the Free Movement of Patients between GP Practices
Patients within the Templepatrick Area can register with the practice. A detailed map showing our boundaries can be viewed at the practice. Should you move to a new address which falls outside the Practice Area you will be required to register with the nearest GP Practice to your address within 1 month, otherwise BSO will be notified and you will be removed from the list.
How to Register with the Surgery
Once it has been established that the individual lives within the Practice Area, they are asked to provide the following documents:
- New Patient Registration Form
- Photographic ID (Passport, Driving Licence, Birth certificate for a child)
- Proof of address (Utility Bill, Bank Statement)
- Brief Summary from previous GP
- Completed HS200 or HSCR1 (see below which applies)
British Citizen moving from another surgery within Northern Ireland, England, Scotland, Wales, please use HS200 form.
Moving from any other country to Northern Ireland, moving back to Northern Ireland after a period of staying in another country or if you haven’t been registered with a GP surgery for the last 6 months, please use the HSCR1 form.
Please leave fully completed registrations in to reception between 8.30am-1pm and 1.30pm-5pm (Thursdays 8.30am – 1pm). If your registration forms and documents are not correct the staff will not be able to accept them. As long as we have the correct documents and forms completed, your registration will be completed within 2 weeks. If your registration request is urgent please highlight this to our admin team.
Please note: all registering patients must provide a brief summary printout from their previous GP. This must include past and present medical problems, medications, allergies and vaccinations. We are unable to register or prescribe medication until this information is received.
Practice Area - Patient Addresses - Outside the Practice area
If your address falls outside the Surgery boundary you will be informed by letter that your address is outside our area and be advised to register with a new GP in your local area.
From the 1st October 2017 if the Practice receive a letter with an address on it which is outside our Practice boundary we will write to the patient advising them that we have received the letter and assume that they have changed address. The letter advises patients that they will need to register with a new GP as the address falls outside the Practice boundary. If the patient has not moved address they are asked to provide 2 forms if address confirmation (e.g. utility bill/bank statement) dated after the date on the letter. These documents have to be provided within 14 days. After the 14 days BSO will be notified of the patient’s change of address and asked to remove the patient form our practice list.
